The "Health History", "Hippa" and "Financial Policy" forms are required by all new patients. The "Chid Health History" form is required by the Adult, parent or guardian of any child under the age of eighteen (18).
If you are unable to print these forms and fill them out completely prior to your initial visit, please show up thirty (30) minutes prior to your scheduled appointment time.
